The Queen's Medical Center-West Oahu Vaccination Clinic

The COVID-19 vaccine is available for individuals 12 years old & older. If you schedule an appointment inappropriately, your appointment will be cancelled or you will be denied vaccination when you arrive at the clinic.

If you are 12 – 17 years-old:

  • Only the Pfizer vaccine is being offered to those 12-17 years old
  • You must be accompanied by a parent or guardian to receive the COVID-19 vaccine.  If you do not have a parent or guardian, please contact our social services department at 808-691-7638 prior to scheduling your appointment

1st DOSE

If you have not been vaccinated

Additional instructions for those ages 12-17 years old:

  • Only the Pfizer vaccine is being offered to those 12-17 years old
  • You must be accompanied by a parent or guardian to receive the COVID-19 vaccine. If you do not have a parent or guardian, please contact our social services department at 808-691-7638 prior to scheduling your appointment. If you would like to schedule an appointment or reschedule your current appointment, please call 808-691-2222.

3rd DOSE

For immunocompromised individuals only

  • Pfizer and Moderna vaccine are being offered
  • You should receive the same type of vaccine for your 3rd dose
  • The Johnson & Johnson vaccine has not been approved for an additional or second dose at this time

BOOSTER DOSE

You have already received two (2) doses of the Pfizer vaccine 

  • Only the Pfizer vaccine is being offered
  • Your 2nd dose of the Pfizer vaccine must be completed at least 6 months prior to receiving a booster dose

Eligible

  • 65 years old or older and long term care facility residents
  • 50 -64 years of age with underlying medical conditions
  • Based on individual benefit and risk who are aged 18-49 with underlying medical conditions
