Blaisdell Vaccination Clinic

The COVID-19 vaccine is available at the Neal Blaisdell Center Pikake Room for individuals 12 years old & older. If you schedule an appointment inappropriately, your appointment will be cancelled or you will be denied vaccination when you arrive at the clinic.

1st DOSE

If you have not been vaccinated

Additional instructions for those ages 12-17 years old:

  • Only the Pfizer vaccine is being offered to those 12-17 years old
  • You must be accompanied by a parent or guardian to receive the COVID-19 vaccine. If you do not have a parent or guardian, please contact our social services department at 808-691-7638 prior to scheduling your appointment. If you would like to schedule an appointment or reschedule your current appointment, please call 808-691-2222.

3rd DOSE

For immunocompromised individuals only

  • Pfizer and Moderna vaccine are being offered
  • You should receive the same type of vaccine for your 3rd dose
  • The Johnson & Johnson vaccine has not been approved for an additional or second dose at this time

BOOSTER DOSE

You have already received two (2) doses of the Pfizer vaccine

  • Only the Pfizer vaccine is being offered
  • Your 2nd dose of the Pfizer vaccine must be completed at least 6 months prior to receiving a booster dose

Eligible

  • I am age 65 years or older and/or am a Long Term Health Care (LTHC)* resident.
  • I am age 18-64 years with underlying medical conditions.
  • I am at increased risk for COVID-19 exposure and transmission because of my occupation or institutional setting

Underlying Medical Conditions at Higher Risk for COVID-19

  • Cancer
  • Chronic kidney disease
  • Chronic lung disease (such as COPD, asthma)
  • Dementia
  • Diabetes
  • Down Syndrome
  • Heart conditions (such as coronary artery disease, hypertension)
  • Liver disease
  • Obesity
  • Pregnancy
  • Sickle cell disease
  • Smoker (former or current)
  • Transplant patients
  • Stroke or cerebral vascular disease
  • Substance user disorder
