Get Vaccinated at Queen's Physicians Office Building (POB) Pharmacies

Please review this guide to help you plan what to bring to your appointment and learn more about your vaccination.

Vaccinations are being administered only to those 12 years old and older for the Pfizer vaccine and 18 years old and older for the Moderna vaccine.  For a complete list of clinics click here.

Effective 5/21/23, POB 3, will no longer provide COVID vaccinations. POBs 1 and 2 and West will continue to offer the COVID-19 vaccine. Please note, POB 1 only offers Moderna and POB 2 only offers Pfizer. POB West will continue to offer both Pfizer and Moderna. Hours remain unchanged.

For the safety and well-being of all, please do not come to our vaccine clinics if you have been diagnosed with COVID-19 in the last 20 days and/or have symptoms of COVID-19 (cough, fever, muscle aches, fatigue, trouble breathing, headache, etc.).

  • There is currently no co-pay associated with the COVID-19 vaccine. 
  • One adult, 18 years of age or older, may accompany individuals who have an appointment. 
  • Face masks covering the nose and mouth, physical distancing, and hand hygiene are required when you come to your appointment. 
  • No animals allowed.

What to Expect During Your Appointment

  • The entire COVID vaccination process may take up to 45 minutes (this includes a 15 to 30-minute monitoring period)
  • Please avoid wearing long-sleeved clothing to help expedite your vaccination
  • No animals allowed

For Your Appointment

  • Please bring:
    • state-issued ID
    • health insurance card / Medicare card
    • completed pre-screening form 
    • COVID-19 vaccination card
  • Verification that you are part of the vaccination group will be checked when you arrive for your vaccination.
  • Face masks, physical distancing, and hand hygiene are required.
  • There is currently no co-pay associated with the COVID-19 vaccine.
  • Print and fill out the COVID-19 pre-screening form below. Bring the completed form to your appointment.

Protect Yourself & Your Ohana

The best protection from COVID-19 is a combination of:

  • Getting a COVID-19 vaccine
  • Wearing a mask
  • Staying at least 6 feet away from others
  • Limiting gatherings
  • Washing your hands often

When the vaccine becomes available to you, we strongly encourage you to get it to protect yourself, your ohana, and the community.
