Please review this guide to help you plan what to bring to your appointment and learn more about your vaccination.

For the safety and well-being of all, please do not come to our vaccine clinics if you have been diagnosed with COVID-19 in the last 20 days and/or have symptoms of COVID-19 (cough, fever, muscle aches, fatigue, trouble breathing, headache, etc…).

What to Expect During Your Appointment

  • You will be receiving either the Moderna or Pfizer COVID-19 vaccine.  Please review the appropriate Vaccine Fact Sheet (linked below) based on the following general guide:
  • We will schedule you to come back to this clinic for your 2nd dose when you receive your 1st dose.
  • The second dose will be scheduled for either 21 days (Pfizer) or 28 days (Moderna) after the first dose to receive proper immunity.
  • The entire vaccination process may take about 45 minutes to 1 hour (this includes a 15-minute monitoring period).
  • Please avoid wearing long-sleeved clothing to help expedite your vaccination.
  • No animals allowed.

For Your Appointment

  • You must bring:
    • state-issued ID
    • health insurance card / Medicare card (if you have one)
    • completed pre-vaccination questionnaire
  • Face masks, physical distancing, and hand hygiene are required.
  • For 2nd dose, 3rd dose, and booster doses: please bring your CDC Vaccination Card to be updated.
  • There is no co-pay associated with the COVID-19 vaccine.
